Care Connection Program
Care Connection provides in-home caregiver placement, family consultations and support for families providing or seeking care. For families wondering what to do next, this program provides a connection to those needing in-home care with qualified independent caregivers who work directly for you. Our goal is to help our residents of our community remain in their own home or assisted living for as long as possible!
Individual Services Include:
- Personal care
- Dementia-specific needs
- Meal preparation
- Grocery Shopping
- Administering Medication
- Diabetes Management
- Physical Therapy Assistance
- Mobility (walking, circulation)
- Assistance with ambulation and transfers
- Twenty-four hour Care
- End-of-life care
Also provided are consultation services to individuals and their families who may have needs other than in-home care, such as Alzheimer's disease and other forms of Dementia, Planning for long-term care, support for family caregivers, and questions about other community resources. Caregivers & Independent Providers Wanted
One of the biggest difference in our Care Connection program is that you are not working for us! As a caregiver or indepdendent provider, you work for yourself and the client. As your own boss, you will earn more money and self-direct your schedule and salary with the client. That also means more flexibility, building a direct relationship with your clients and taking more control of your own life.

Caregiver in the Spotlight
Ever since she was a young girl growing up in Perth, Australia, hanging out with people who needed care was very much a part of Sandy Lestenkof’s life. Many family members were in the healthcare field including her dad who helped build a facility for people with disabilities. “It was very natural to hang out and play with these people.
